Replace, reapply and update your disability parking permit details
Multiple permits
From 15 September 2025, all disability individual parking permit holders may hold up to a maximum of 3 disability parking permits at once.
You can apply to hold additional disability parking permits:
- online
- by completing ‘Part 1 – Applicant details’ of the Australian Disability Parking Permit—Mobility impairment application Individual) form (F4814).
Replace or request additional disability parking permits
You can use the disability parking permit application form or online service to request a replacement for a lost, stolen or damaged disability parking permit or to request additional disability parking permits.
All individual disability parking permit holders may hold up to a maximum of three disability parking permits at one time. You can use the disability parking permit application form or online service to request an additional permit.
You don’t need to see your health professional to certify an application to replace your permit or to receive an additional permit.
Please keep a record of all your disability parking permit numbers to have available when applying for an additional or replacement permit. When you request a replacement for a permit, the original permit may be cancelled to ensure lost or stolen permits are no longer valid.
The Department of Transport and Main Roads will not issue more than 3 permits to an individual at one time.
For individuals
To replace an individual disability parking permit or request additional permits, you can:

For organisations
To replace a disability parking permit issued to an organisation, you can:
To apply in person or via mail, complete the Australian disability parking permit application for organisations (F4881) form and lodge your application in person or by post, using the details on the form.
Reapply for your Australian disability parking permit
We'll send you a notice in the mail about 6 weeks before your permit expires.
To reapply for your Australian disability parking permit you need to meet the eligibility criteria.
You don’t need a health professional to certify the application if you have a 5-year Australian disability parking permit and your application is received within 3 months of the expiry date.
If you have a permit for a temporary condition (6 to 12 months) you will need to apply as a new applicant when your permit expires.

Reapply for your red permit
Red permits are being phased out from September 2025, with red permit holders to be transitioned on to Australian disability parking permits.
It is important to note that you will not be disadvantaged by this transition and that your access to designated disability parking bays will now include both off-street and on-street parking with an Australian disability parking permit.
As you have held a red permit for at least 5 years, you don’t need a health professional to certify the application if your application is received within 3 months of the permit expiry date.
We’ll send you a notice in the mail about 6 weeks before your permit expires, inviting you to apply for a free Australian disability parking permit.
Renew an expired permit
You need to apply for a new permit if your permit has expired. You may get a fine if you continue using an expired permit.
Change of name
To change your name on your permit, complete a Change of customer details form (F4214) and take the form to your nearest transport and motoring customer service centre.
Change of address
Update your address in person, over the phone, or by mail.

- By phone: Call us on 13 23 80. You will need to provide your name, date of birth, old and new address and disability parking permit number.

Notify us of a deceased permit holder
You can let us know a permit holder has passed away by providing a copy of 1 of the following documents:
- photocopy of the death certificate
- funeral notice
- coroner’s report
- Certificate of Authority issued by the Public Trustee under the Public Trustee Act 1978.
